妖精大戦争　～東方三月精 ovvero Yousei Daisensou ~ Touhou Sangetsusei, il significato letterale è Great Fairy Wars ~ Le tre fate orientali . Nuovo spin off stavolta realizzato per il manga Strange and Bright Nature Deity (difatti le illustrazioni sono a cura dell'illustratore del manga Makoto Hirasaka), si focalizza su Cirno e sulle tre fate pestifere.
Si presenta con le stesse caratteristiche di un Vertical Shooter tuttavia con alcune particolarità:
Nel gioco controllerete una Cirno che si scaglia contro le altre fate con grinta e sicurezza, scaturita anche dal fatto che la sua abilità speciale sta nel congelare i danmaku vicini i quali, congelati, trasmetteranno lo stesso status ad altri vicini a loro e così via fino a creare un effetto domino che vi farà guadagnare più punti e più potenza. Questa particolare abilità è l'aspetto fondamentale del gioco, il quale si manifesta se il vostro conteggio sopra la testa arriva al 100%. Più danmaku assieme riuscite a congelare, più il vostro livello (segnato in basso a destra) si eleverà rendendovi più potenti.
La vostra vita è calcolata in percentuale, l'opzione si trova in alto a destra. Se verrete colpiti, la percentuale scenderà pericolosamente ma congelando danmaku e sparando, questa si alzerà nuovamente. Avrete inoltre a disposizione delle bombe (sono i fiocchi di neve situati in basso a sinistra dello schermo) che congeleranno ogni danmaku sullo schermo, ripulendolo.
Un'altra particolarità rappresenta il metodo di visualizzazzione della barra vitale nemica: questa è a forma di cerchio, posta attorno allo sprite del boss.
Il gioco è composto da soli tre stage più un extra con un boss inaspettato! Le strade da percorrere sono diverse, a seconda della scelta potrete incontrare per prima una diversa fatina (tenete conto che comunque le incontrerete tutte e tre durante il cammino, cambia solo in che ordine) e si dividono in Route A, Route B e Route C, con un percorso alternativo per ogni route che è possibile scegliere all'inizio del secondo stage. 妖精大戦争　～東方三月精 (romanized: Yousei Daisensou ~ Touhou Sangetsusei, meaning "Great Fairy Wars ~ Eastern Three Fairies") is a vertical danmaku freezing game and is the 12.8th game in the series. It is a continuation of a story in Strange and Bright Nature Deity and is the only danmaku game by ZUN where he did not draw the portraits himself. The portraits were drawn by Makoto Hirasaka.
The game plays somewhat like a fusion of the Shoot the Bullet series and the Phantasmagoria series, combined with "standard" Touhou gameplay. Cirno is able to shoot regularly, but also possesses a new "freezing" technique that adds a new twist to the game. Similar to Aya's camera, Cirno has a counter that fills up during gameplay. To activate this technique, hold and release the shot button. When activated, Cirno's freeze attack will attack the immediate area directly in front of her, and freeze any bullets it comes in contact with for a duration proportionate to the percentage of charge before vanishing afterwards. When other bullets come into contact with frozen bullets, they too will freeze, allowing for large chains to be frozen in this way. Points and bombs are awarded for freezing large numbers of bullets.
Like the main-series games, Fairy Wars contains full stages each home to their own specific midboss and boss. However, Cirno is the only playable character, and there are only three stages during a playthrough instead of six. On the other hand, though, there are six different "routes" the player may choose to take, depending on which order they choose to play the three stages in. There is also a seventh extra route which must be unlocked before being playable.
